Shoulder Impingement: Symptoms, Causes and Exercise

Short answer: Shoulder impingement is the tendon and bursa being pinched under the bony arch when the arm is lifted. It is usually linked to weak shoulder-blade control, posture and repetitive overhead activity. Physiotherapy corrects the mechanics that cause the pinching; most cases heal without surgery.

Symptoms

  • Pain when lifting the arm between 60 and 120 degrees
  • Night pain, unable to lie on the shoulder
  • Difficulty combing hair, fastening a bra, reaching a back pocket
  • A sense of weakness

Causes

  • Rounded-shoulder posture
  • Weak shoulder-blade muscles
  • Overhead sports and repetitive work
  • Rotator cuff tendon problems

The physiotherapy process

  1. Assessment: which movement, which angle, which muscle?
  2. Shoulder-blade control and rotator cuff strengthening
  3. Manual therapy and chest stretching
  4. Gradual return to sport or work
